We are currently recruiting for a Community Link Worker to promote social inclusion by helping service users identify and achieve their goals. Provide a range of community based groups/ sessions and promote access to a range of activities within the wider community.

To harness the strengths and abilities of local communities to promote their health, wellbeing and independence through the provision of a Medway Community Connect service.

Ensuring the participation of people who experience health inequalities including people with care and support needs in a variety of community activities.

Identifying and developing connections with local assets such as organisations, community groups, businesses, community centres.

Embedding actions to grow community resilience in Medway into the work of Integrated Social Care teams at locality level.

In conjunction with the Integrated Practitioners, develop and support a range of purposeful, enjoyable and interesting social activities relevant to the needs of people experiencing significant impact on their wellbeing.

Engaging with communities and supporting them to address local issues using strength based approaches. Working with communities to map their assets

Provide information to individuals on the range of community preventative services including universal services

Essential qualifications

A level 3 qualification in Health, social care or voluntary work
