Social Worker - Integrated Discharge Team
An exciting opportunity has arisen for Social Workers who are highly motivated to work within our busy Integrated Hospital Discharge Team.
The role involves working with new and existing service user’s service users where we may offer support, advice and guidance to facilitate hospital discharge.
The service is delivered using a strengths based, personalised approach. The role requires multi-disciplinary working ensure the client is at the centre of discharge planning.
The successful candidates will be a qualified social worker ideally with knowledge and experience of working with adults across service user groups including learning disability, physical disability, older adults and mental health.
Comprehensive knowledge and experience of the Mental Capacity Act and Safeguarding is essential. Previous hospital discharge experience is desirable however we will offer a full and comprehensive induction package to ensure new staff are supported.
The role will include assessing vulnerable service users and supporting them to meet their desired outcomes, these assessments will take place in a hospital setting which includes both acute and non-acute settings. The successful candidates will be required undertake Safeguarding cases when required.
The Hospital Discharge Team currently functions over a 6 day period, Monday through to Saturday. The discharge team operates between 8am to 8pm Monday to Friday, 9 till 4 on Saturdays. There will be a rota in place for staff to cover the office during these times.
