A social worker on the Shared Lives team is responsible for the recruitment, support and development of carers.  Managing a varied caseload of Shared lives carers with regular contact and support and monitoring visits every 10-12 weeks minimum, to ensure the required standards are met and maintained. You will encourage and support Shared carers to enable the individuals placed with them to achieve the best outcomes. 

You will complete assessments of new applicants in line with policy and guidance, and produce comprehensive reports of a high quality, to be submitted and presented to the Approval Panel within the appropriate timescales.

The ongoing care planning and the assessment of risk is a key part of the role, and clear and prompt recording and documentation is essential.

With team colleagues you will be involved in recruiting new carers by attending recruitment and awareness raising events and supporting existing carers at regular support groups.
