Specialist Health Improvement Practitioner
Please note this is fixed term contract to cover maternity leave for 1 year.

The Supporting Healthy Weight Team are recruiting to this role that will support those complex, most vulnerable young children and their families/carers to lead healthier lives.
The project will encompass a whole family approach, and offer practical support for those who may be living with increased levels of poor mental health, who are physically inactive, practice poor eating habits, and who may struggle with parenting issues. This role will be key to getting to understand the behaviours and lifestyle of the family.
The postholder will also be responsible for the co-ordination of multidisciplinary team meetings and will be the key point of contact with the psychotherapy service.
Partners may include the following: Social Care, Community Health, School Nurses, Public Health, Dieticians, Mental Health teams and staff at the Acute trust such as Paediatricians.
- Registered Nursing qualification or equivalent clinical professional qualification
- Qualification or Experience of working with children and families
- Evidence of continual professional development.
- Sohilhull Approach parenting training.
- Motivational Interviewing and Behaviour change training.
